Case Manager

To support clients and families by assessing needs, coordinating services, providing referrals, and monitoring progress. The ideal candidate will serve as an advocate and resource, helping clients access appropriate community, educational, social, and healthcare supports.

Requirements:
Master’s degree in a mental health, social work, counseling, or related field, or equivalent experience
3–5 years of experience preferred
Strong communication, organizational, and time management skills
